Heather what I do when I have to make a thread loop for a button is start out with your base. Double thread your hand sewing needle and make a loop where it is needed the size you need and tack it so you don't accidently alter the size when you start making stitches over the thread. Don't cut the thread after tacking the loop. Now start making the buttonhole stitch over the entire thread loop, keeping them side by side. I hope I explained this so you can follow. This does make a very attractive and durable button loop.

By the way, this method can be used to make a swinging snap to go at the top of a zipper too. You just make a straight line to and from the snap (back and forth. Then start making your knots at the snap end working back toward the garment. This is a really nice look at the top of the zipper. The knots? You make a loop around the thread loop and then take your needle through the loop. Always go from the same side of the loop (inside or outside); it makes a lovely finish.
